First aid for suspected hemothorax includes stopping external bleeding (if the wound is open) and stabilizing the condition. The victim is placed in a semi-sitting position with a tilt to the side of the injury - this facilitates breathing. Applying an occlusive dressing to the chest wound is mandatory to prevent air infiltration. It is strictly forbidden to remove foreign objects from the wound or attempt to drain the pleural cavity without equipment - this will increase bleeding.

For transportation, it is important to provide oxygen support (if balloons are available) and infusion therapy (crystalloid solutions) to control hypovolemia. Analgesia is performed with caution: opioids are contraindicated in respiratory depression, ketorolac (10-30 mg intramuscularly) is preferred.

In hospital treatment includes drainage of the pleural cavity by thoracostomy or thoracentesis, as well as surgical control of the source of bleeding. If more than 1.5 liters of blood is lost or bleeding persists, thoracotomy is required.

Thus, hemothorax requires rapid diagnosis and emergency measures. In the SWO zone, where evacuation time is often prolonged, competent primary care plays a key role: wound sealing, respiratory and circulatory support.
