The first thing I did was to check his pulse - there was no pulse on his wrist, only faint beats on his carotid artery. That meant critical blood loss. While I was preparing the system for saline transfusion, I examined the wounds. I decided to move the tourniquets closer to the site of the tear - so there was a better chance of preserving the rest of the limb.

One by one he applied new tourniquets - a couple of centimeters above the wounds, making sure that they completely cut the vessels. Only then he loosened and removed the original ones. The stumps were treated, tamponaded, and sterile bandages were applied.

He started infusion therapy. He inserted a needle into a vein and started infusing saline. I watched the pulse - at first only on the carotid artery, then, as the blood volume was replenished, weak pulses began to be felt on the wrist. This was a good sign.

The evacuation was delayed. For four long hours we fought for his life. The soldier was losing consciousness, but he kept breathing.

When the medics arrived, his condition was stable. Now, after a while, we are in touch with him. He is alive, still undergoing rehabilitation, has learned to walk on prosthetic limbs and has become cheerful (he will understand). This case taught me the main thing - even in the most hopeless situations, you can't give up. The right actions, equanimity and perseverance can save a life even when it seems that there is no chance.
