Electrocardiography in pulmonary embolism registers signs of overload of the right heart: McGin-White syndrome, inversion of the T in the right thoracic leads. If cardiogenic shock develops, echocardiography is indicated to visualize thrombi in the heart cavities.

Emergency care includes immediate administration of 10000 units of sodium heparin intravenous sodium heparin by IV infusion to prevent further thrombosis. If pulmonary embolism is confirmed, thrombolytic therapy is used: 4.5 million units of streptokinase intravenous drip over 30 minutes or 100 milligrams of alteplase over 2 hours. For pain control, 10 milligrams of morphine is administered intravenously in fractions. In case of arterial occlusion of the limb, emergency embolectomy with a Fogarty catheter is performed.

Anticoagulant therapy is continued with sodium heparin 1000 units per hour intravenously under control of ACEI with transition to warfarin 5 milligrams per os daily. Monitoring of the condition includes control of oxygen saturation, ECG and diuresis.

Prevention of this complication requires strict adherence to the technique of gradual tourniquet removal with a preliminary injection of 5000 units of sodium heparin intravenously 10 minutes before decompression. When the tourniquet application time exceeds 120 minutes, catheterization of the artery distal to the tourniquet before its removal is necessary to control the embologenic material. Training in the rules of tourniquet removal with practice on simulators is a mandatory element of training of tactical medicine instructors, because prevention of thromboembolic complications directly affects the survival rate of the wounded with prolonged limb ischemia.

In case of signs of increasing neurological deficit or respiratory insufficiency, it is necessary to correct the body position and transportation parameters.

Training in transportation rules is a critical element of tactical medicine instructor training, as the quality of care at the evacuation stage directly affects the further effectiveness of treatment and the possibility of full rehabilitation of the combatant. Regular training in the proper extraction and transfer of wounded personnel using special equipment should account for at least 20 percent of the training time in the tactical medicine program.

The only method of treatment is emergency surgery - laparotomy or thoracotomy, during which the organs are returned to their anatomical position, and the diaphragm defect is sutured. Prior to surgery, a broad-spectrum antibiotic, such as ceftriaxone one gram intramuscularly, is administered to prevent infectious complications. Delayed surgery is fraught with impingement of diaphragmatic hernia, necrosis of impinged organs, the development of severe sepsis and inevitably leads to death. Thus, the basis for saving life in diaphragmatic rupture is not rapid diagnosis on the battlefield, which is often impossible, but high alertness of the medic, competent maintenance of vital functions and the fastest possible referral of the wounded to the surgical stage of care.
